Role Responsibilities

  • Manage a primary caseload, ensuring continuity of care for assigned patients

  • Create a supportive and empathetic environment to enhance the patient’s clinical experience by:

    • Preparing for each visit by reviewing patient charts in eClinicalWorks (eCW)

    • Warmly greeting and escorting patients to their visit rooms

    • Conducting nursing assessments, including interviews, vital signs, weight measurements, and other relevant clinical tasks
