Manage a primary caseload, ensuring continuity of care for assigned patients
Create a supportive and empathetic environment to enhance the patient’s clinical experience by:
Preparing for each visit by reviewing patient charts in eClinicalWorks (eCW)
Warmly greeting and escorting patients to their visit rooms
Conducting nursing assessments, including interviews, vital signs, weight measurements, and other relevant clinical tasks
Assist providers by sharing relevant patient information from assessments and setting up TeleHealth equipment when necessary
Monitor patient schedules, ensuring completion of both counseling and medical appointments

Safely operate the mobile treatment vehicle, performing pre- and post-trip inspections
